Communication and incident reporting are inconsistent, staff failed to document to provide clear guidance to upcoming staff about what require attention. The work environment has become discouraging due to excessive workloads, poor leadership involvement, and a culture focused more on speak about others than provide actual solutions. Residents and employees are both affected by the lack of accountability, staffing support, and professional management standards.
